Because parents’ schedules don’t always revolve around school hours, snow days, or holidays, our before and after school care programs provide children with a safe place to stay before and after school hours. Programs consist of homework assistance, recreation, arts and crafts, and community service activities for middle and intermediate students; and fun learning activities, staff directed homework assistance, and quiet reading time for children in grades K-4. Daily snacks are provided with all programs.
IMPORTANT INFORMATION:
- All children registered will need the OCFS Daycare Enrollment form (below) filled out and signed along with the most recent copy of physical and shot records.
- IF your child has ANY allergy to ANY thing and it is noted on their physical, WE MUST have a separate anaphylaxis form for each allergy. This must be updated annually. NO EXCEPTIONS.
- There is also a medication consent form (below) that will need to be filled out by both the parent and physician.
- It is not advisable to send screen shots of the forms, this may slow down the processing of your registration, which you may have to resend in another format.
- Sending all documents in together will facilitate the approval process.
